Dyslexia Card for Teachers - from Dyslexic Advantage. Actually, 1 in 5 students are dyslexic, but other than that, this would be a good card to give to your child's teachers. Maybe have something on the other side about your child specifically.
I have parents fill this out at the beginning of the school year so that I know the best way to contact them throughout the school year. I also added a log to keep track of parent communication. This is especially helpful if there are consistent concerns or a long term sub.
Snail Investigation.... this is a great anchor chart for inquiry. Students write what they see, think, and wonder about the question. They can put sticky notes up so that the anchor chart can be used again.
